Worth NGC grading? 1988 nickel 0

Hi I'm new here I'm curious if I have found a coin worth getting graded? I was looking through change and this nickel caught my eye . On the reverse it appears to have a head from a different denomination on it. It look to be around the same size as Washington's head on a quarter. Same size and shape. So I'm asking 4 questions: does this appear to be an error? If so what would it be called? Does it appear to be gradable? And would it be worth getting graded?......Thank you for your time and opinions!

The only head I see is Washington’s head on the Obverse. The reverse is just a stain. Value 5 cents
