The coin on the left is just more fully struck in the rims. This could result from differences in the milling of the collar which directs metal flow toward either the coin's center or its peripheries, but rarely both on a currency strike.

There actually is a Medium Relief coin made for circulation, and just a single piece is known today. Note that the legends and numerals are bolder than on the o.p.'s coin:
