Don't know how often it happens, but it has been going on since the creation of the registry sets.  It died down some for awhile and then picked up again when fake slabs appeared so they could put valid numbers on the fake slabs.  That was one of the things that makes NGC's certification verification better, they started putting images of ALL the coins they certified up with the verification at about that time.  You could not only verify the certification, you could also verifiy that the coin/slab was good.
