Would this coin receive a straight grade even with the black spots?

In my opinion yes it would straight grade for sure. Although spots like that may limit the grade.

I think the answer is yes, but are they spots or stains?

Isle of Man sovereigns are known, at least to me, for having a high probably for red spots and in this case black. They are not a stain - and have been in an air tight for years.
