Here is a sample that I picked up some time ago and

Sample Slabs by Cameron K. Has it listed as type 1

Sample Slabs book by David Schwager has it catalog as NGC-010-2-4

I highly recommend David's book. A lot of the really weird stuff is in there...

 

This is one of my favs (Schwager NGC-Br2-4-1):

 

 

Based on the slab style and coin type I was able to trace it as most likely from the 2001 ANA Convention in SLC.
