Debris in the slab & marks on the insert

I recently bought a coin in ngc holder. When i received it I noticed there's a white debris on the coin inside the slab which looks like the white insert material.

Also the insert seems not in suitable size with the coin. There are some black marks left on the white insert, which appears to be the patina of the coin. It seems someone tried very hard to thrust the coin into the slab...

So.. is it a normal case for ngc's slab?.. should I change the slab..?

So do you think the white debris is removable if I send the coin for reholder? I'm not sure if it will gradually undermine the coin's surface. And I don't know if a coin conservation is needed as i don't really want to clean the coin but the very annoying white thingy.
