That's not an error. It was intentionally made.

The mint worker who intentionally made this piece somehow smuggled it out so that they could sell it. 

The mint does sell regular waffle cancelled coins, for a large premium.

The mint sells regular waffled coins as scrap metal.  Actually the GSA handles the sales.  And frankly that is how this piece could have gotten out, as scrap metal.
