Oversize Holder Submission

On the NGC Services and Fees page it explains that to have a coin placed in an NGC oversize holder it must be submitted on a separate submission form.  I have two different items that I would like graded and put into oversize holders.  Do I need to have two separate submission forms or can I put both of them on the same one?  Thank You.

Thank you for your question. Over-sized coins must be placed on a separate submission form from your normal sized coins. If the over-sized coins qualify for the same grading tier they can be placed on one form together.
